Where Do I Start? Online CGFM Application Form CGFM Application Options Download CGFM Work Verification Form CGFM Process

Choose one of the three ways to apply for the CGFM Program:

1. Fill out the online CGFM application form.

2. Download the CGFM application form in Adobe Portable Document (PDF) format, and fax or mail it to AGA.

   You will need Adobe Acrobat Reader to view and print the form, which you can download free by clicking the button below.

3. Request a paper CGFM application by filling out the CGFM application request form, and then fax or mail the application form to AGA.
