Chapter CGFM Award
Please use this form to complete your nomination. Nominators should complete Sections I - V and submit the nomination by the deadline. (These materials may be reproduced and distributed freely.)
Purpose and Overview
- Each year, AGA presents crystal awards to recognize AGA chapters
that have done an outstanding job in promoting the CGFM program and its
members, other accountability professionals and their community.
- Receipt of the award reflects that a chapter is willing to and had
dedicated significant time and resources to further the recognition and
integrity of the CGFM Program.
- AGA chapters and National Boards will make nominations. The Chapter
President or his/her designees shall approve the nomination.
- AGA presents one award per chapter membership group.
- AGA will nationally recognize the winning chapter(s) at a special
awards ceremony during the Annual Professional Development Conference
and Exposition.

Criteria and Evaluation -
The National Awards Committee will
use the following criteria to evaluate and rank nominees.
Therefore, for each key factor listed below, describe in specific
terms, the work or contribution that justifies the nomination.
1. Describe in detail the chapter’s annual plan and chapter
communications for promoting the CGFM program, including any press
releases, proclamations, and Chapter newsletter articles. (30
points)
2. Show
evidence of the comprehensive, diverse, and innovative efforts
and/or successful results that benefit AGA and the CGFM program by
- Obtaining formal, tangible recognition of the CGFM by government
agencies or other entities
- Providing specific opportunities and incentives to individuals for
obtaining their CGFM,
- Increasing the number of Chapter members who earned the CGFM
designation,
- Maintaining the CGFM retention numbers in the Chapter, and/or
- Increasing the value of the CGFM designation or of AGA membership.

For all co-sponsored certification promotional events with
another AGA chapter or professional association (e.g., IIA, ISACA,
ACFE, etc.) listed in no. 2 above, identify the co-sponsor and
describe in detail the chapter’s role/participation in offering
these events. (10 points)

How to Prepare and Submit Your Entry
Deadline
- The original nomination package must be received by 5:00 pm Eastern time on Friday,
March 6, 2009.
- Late nominations will be encouraged to resubmit the following year – they will not be returned.
- All applicants will be notified of the National Awards Committee's decision approximately 4-5 weeks after the deadline.
